Why is it called “Swing Bed”?
Swing Bed programs were designed by the government to allow patients to “swing” or move from an acute level of care to a skilled rehabilitation level of care, but remain in a hospital setting.

Sylvan Grove Hospital’s Swing Bed program blends skilled nursing with rehabilitation in a caring, personalized setting to help patient’s recover and gain strength to return home.

Who needs the Swing Bed program?
Swing Bed is an extended care option when a patient has recovered from an illness or injury and no longer needs acute hospitalization, but they are not able to go home yet.

Some of the types of patients we see include those who are:

  • Recovering from joint replacement or other types of surgery
  • Recovering from stroke, cardiac or respiratory illnesses, or other medical conditions
  • Have wounds that need long-term antibiotic therapy or that can’t be treated at home or in an outpatient setting

Where is Sylvan Grove Hospital located?
Sylvan Grove Hospital is located at 1050 McDonough Road in Jackson, Georgia, which is southeast of metro Atlanta and near Spalding, Henry, Lamar, Monroe and Jasper counties.

How do I get admitted into the Swing Bed program?
Speak with your physician or case manager, who will refer you to our program. Once they have sent a referral, our Swing Bed coordinator will visit you at the hospital to meet with you and your family members and conduct an assessment to ensure you will benefit from our program.

In addition to a physician referral, you will need to have been in an acute care hospital for at least three nights in a row before being transferred to the Swing Bed program.

I have been discharged home from a hospital, but having difficulties recovering on my own or on an outpatient basis. Can I still be admitted into the program?
Yes, as long as you have stayed in the hospital for at least three nights in a row within the last 30 days.

What is a typical day like at the program?
Since each individual is unique, our care team will set personalized treatment goals for your stay. Your treatment plan will be tailored to your needs and goals.

During the day, you will receive therapy and treatment based on your condition and recovery goals. You may receive physical, occupational and speech therapy, respiratory therapy, IV therapy or other skilled nursing care. Similar to the hospital, you’ll receive around-the-clock nursing care and your care is overseen by one of our staff physicians.

When you are not in therapy, you can spend time visiting with family and friends, playing cards or board games, participating in one of our planned activities or just relaxing.

How long can I stay?
You can remain in the program as long as you have therapy and rehabilitation goals to meet. Once you have reached your goals, some patients are able to return home and some patients may need to explore additional care options. The care team can help in the process by providing recommendations and alternatives for future care when necessary.

Is the Swing Bed program covered by insurance?
Yes, the Swing Bed program is often covered by Medicare as well as many private insurance plans.
